My notebook has 4 settings: performance, balanced, silent and energy saving. Why is it really silent on "Save Energy" and on "Silent" you can clearly hear the fan?

With energy saving you have the worst performance.

It is possible that the laptop relies on passive cooling to save energy, i.e. Fans off
