Either plug the headphone output into the PC's line-in jack via a jack-jack cable and then send the sound to the PC, or through an audio networking solution that distributes audio over the LAN to other computers with appropriate software.
You can also just take a mixer.
What exactly do you mean with a jack-jack cable?
3.5mm jack to 3.5mm jack… This has a plug on both ends, as on the headphones. So you can connect the laptop headphone output to the PC in the line-in (usually blue socket).
