The animations were simply not lip-synced. Given the large number of dialogues, this would also be too difficult to implement.

There are technologies that can sometimes hide it better, sometimes less, but in the majority of them, adapting such animations remains a greater challenge, especially in the overall game with facial expressions and gestures. And even if it does not seem that way, CD-Projekt was not yet a Triple-A developer and with a comparatively small team and rather low budget at the Witcher 3 delivered a lot that you would not have expected. The fact that compromises were necessary here and there can't be avoided on the order of magnitude.

You can repair very little. This is programmed into the game. The easiest way to change the language is to choose one that overlaps with the animations. At the time, however, I did not perceive it as dramatically as you describe it. The title has been around for four years now.
